SPECIAL ALCOHOL & DRUG ASSESSMENT

	Karen Hembrook MSSW, LCSW, CADC III, SAP handles the special alcohol and drug assessments.
	Ms Hembrook has experience as clinician, program manager, and lead counselor in various facilities as well as an experienced addictions therapist. Ms Hembrook will provide SAP evaluations per DOT 49 CFR part 40 Ms Hembrook will provide services to individuals and respective companies who will refer these individuals who have violated a Department of Transportation regulation covering the following provider types:
	Federal Motor Carrier Safety Administration Operators of Commercial Motor Vehicles 26,001 or more Operators who transport 16 or more occupants Operators who transport hazardous materials
	Federal Railroad Administration Engineers, Conductors, Signalmen, Operators, Dispatchers and Switchmen.
	Federal Aviation Administration Persons who perform flight crewmember duties, flight attendant duties, flight instruction, aircraft dispatch duties, Aircraft maintenance, ground security, coordinator and/or air traffic control duties.
	Federal Transit Administration Revenue Vehicle operators, maintenance, control or dispatch Cornmercial Drivers License, non revenue operation or armed security duties.
	United Stated Coast Guard A person on board a vessel acting under certificate of registry or merchant mariner's document or engaged or employed on board a U.S. owned vessel having license or certificate of registry, or merchant mariner's document.
	Research and Special Programs Administration A person performs on a pipeline or liquefied natural gas facility an operation, maintenance, or emergency-response functions.
