FINANCING YOUR THERAPY
Insurance fee reimbursement forms are filed as a courtesy twice per month as a service to clients. This requires the client's policy information, social security number, date of birth, insurance company name and address, group and individual identification numbers, as well as an authorization to submit claims forms signed by the client for our files. In addition, some insurance companies require a physician's written referral for psychotherapy service. In such cases, we will not file claims with these companies until we have a copy of the referral. |

Billings are printed according to the therapist seen. So, a couple or family being seen by two therapists will be billed for each therapist involved in each session. |

The ultimate responsibility for the charges belong to the client. The Clinic encourages each client to pay something each session, the amount to be worked out with the therapist. We would appreciate payment of the agreed co-payment fee at each session. Clients will receive monthly statements notifying them of the balance due on their accounts. Any amounts outstanding over 90 days, including payments expected from your insurance company, will be due immediately from you. Collection agency services may be arranged by us if you allow your account to become delinquent. |
