  We have several types and levels of Alcohol & Other Drugs Assessments at JPC.

  Accent1Who can do these?
Only people with a CADC certification or SAP. On staff, these are, Bill Hollingsworth, Wendy Hein-Drew and Lynda Richter.

  Accent1Type #1 Underage Assessment for Alcohol.
This usually takes 30-45 minutes and the Rock County agreed upon fee is universally $75, paid at the time of assessment.

  Accent1Type #2 Regular A & D Assessment for anything else.
Get back into School, IDP (Intoxicated Driver Program), Probation/Parole Department, and/or Attorney/individual requests. $140 per hour.

  Accent1Type #3 SAP Substance Abuse Professional
William Hollingsworth. Appointment will be made through JPC. These special assessments take about 2 hours.

  Accent1Requirements for Completion of Treatment:
  1. Attendance at Scheduled Group Therapy Sessions Attendance at Scheduled Individual Therapy Sessions Client will agree to remain substance free during term of treatment Client will agree to submit to UA's during course of treatment After IOP the client agrees to continue with after care treatment Client will agree to Attendance at a 12 step program Client will create a relapse prevention plan with action steps Client will create a life story exercise. Damage to life from use/abuse/dependance Group will have input into client's readiness to graduate and/or move to aftercare/discharge
  2. Client will fill out group evaluation and a brief paragraph as to what was gained from the group.
